Patient Assessment Card
The manual handling instruction card is a communication tool designed to summarise and display the results of the patient assessment to all handlers. It is divided into four activity categories: bed mobility; in and out of bed; sit to stand and mobility. Choices range from independent, to assisted, to fully dependent; and includes a range of equipment choices in each category. Options are in line with minimal lifting programs. Information is displayed in words and easy to read symbols.
The card is laminated. Circle or tick your choice with a water soluble pen so that the results can readily be changed with each reassessment and each new patient.
Should you wish to make your own card, 17 symbols are also available on disc or CD.
